Dane-Elec Micro SD Card: Format Failure? Please help!?

Okay so here's the story:

I have a Micro SD card, with an adapter.[From Dane-Elec] I wanted to put songs on it, and transfer them to my phone, a Razor. So I tested it to see if it would work, and put 3 songs on the card, and moved them to my phone sucessfully.

Then I decided to move all my songs at once, about 50 total. So I moved them all on there. But when I put it in the phone, and went to check for the songs they weren't there.

I tried to find out what happened, and it said it needed to be formatted. So I said yes, knowing I still had copies of my songs, so I was willing to loose the data.

I thought it would work, but when I stuck it in my computer, it just made some noise. I went to check for it, but it showed up no where. [The SD folder]

Also, usually when I put it in the computer, a thing would pop-up saying which way do you want to open up files. But it didn't this time.

So I put the micro in my phone, tried to format it again, and it said, FORMAT FAILURE.

So how do I fix it? Can I do it, or do I need someone else to so it?
