How do i do this cosmetic clinic?

You're getting ahead of yourself a little....
What you should do first is learn what you're letting yourself in for.
I suggest you download a copy of the HBIGDA Standards of Care; you can find them here http://www.wpath.org/publications_standards.cfm (these ARE the HBIGDA SOC, even though WPATH have claimed them as theirs), and READ them. You would also benefit from taking a look around this website; http://www.tsroadmap.com/index.html - it contains a lot of very useful information.
The basic process is;
You visit your GP/family doctor, tell him or her you think you might be transsexual, and ask to be referred to a psychiatrist (NO, this doesn't mean you're mentally ill; it's to officially ensure that you're not).
If your psych agrees, he or she will refer you to a gender specialist, who will also examine and assess you.
If the gender specialist agrees that you ARE transsexual, he or she will begin your therapy; this usually begins with a period of counselling.
After a time (usualy a minimum of three months) in counselling, you may be prescribed hormones.
You will probably be expected to begin social transition (living, working and socialising in your "target gender" 24/7) at around this time. During this time, you will have several other things to deal with; facial and body hair removal, and voice therapy (for trans women), chest reconstruction (for trans men), changing your name on your ID and legal documents (for both), and a few others.
You will have to do this for a minimum of one year (two, in the UK) before you can be considered for surgery.
The whole process typically takes 5-10 years; it can be done in less, and it can take much longer, depending on your personal circumstances. It's not a quick process, and it's not cheap or easy either.
But, to answer your question, the first step is to visit your doctor. You can't do this without him or her.

go to your local GLBT center and start talking to the employees they should be able to give you more information ... if theres not a local center you can always look online for more info
