Surname is the last name (or "family" name).

Given name is the first name.

English name is a name that Asian people often have (usually a name they have chosen themselves) as well as their Asian name.

    e.g.

    Fong (given name)

    John (English name)

    Lee (surname)

It would be written as "Fong (John) Lee"

Not everyone has a middle name.
