applying for long birth certificate?
On the form it asks for "address of birth". Does this mean the address of the hospital or the address where i lived when i was born?
I know its in wrong category couldnt get answers anywhere else!!

considering that i just just filed one it means the address of the hospital if you don't know the address or aren't sure just put city and state

The hospital or address if born at home . Usually parents get a long one ,, stating name of father and so on .. Wonder why you have a short one ,,

address of birth usually means the hospital's name and address where you were born...
specially since lately there are home births more common that are assisted and in midwife's birthing units... thats why...
what country are you filling the certificate for?

Suggest that you supply both. The name of the Hospital and your parents' address at the time. That will make their search easier and likely quicker.

isn't that seriously the dumbest question on an application...i just put the city of the hospital i was born at !!!
