I have never been ABSOLUTELY SURE what Petrucci was talking about EXACTLY but I know theres a spoken dialog in the song that mentions "illness" and the lyrics also suggest hints of Schizophrenia.
There another song called Panic Attack more toward Anxiety and Paranoia that I really like

Loco Astrum~** This is such a unique question, at least I havent heard it before. :) Hmmm. This is difficult. I have rapidly odd changing moods (I dont think im bipolar though) I love love songs but I am depressed at lot, and I have my happy moments and I have a quick temper. Probably....oh Lord..... Confessions Of A Broken Heart (Daughter To Father)-Lindsay Lohan I can relate a lot, unfortunately.

It wasn't a song, more of a nursery rhyme about schitzophrenia.

I'm a schitzophrenic

It's silly, I know... but you asked. :)
As far as actual songs go, I'de have to consider Pink Floyd's "The Wall" as a top song/album of someone dealing with a personality disorder.
