Check your fluid it might be contaminated.
Does it get any better once the car is warmed up?
I had this problem in my 95 Suburban once, when it was cold it would take a while to shift through the gears. But once it warmed up it was fine. I switched the fluid out with Valvoline and it fixed my problem.

It sounds like the sun gear in the transmission failed. This is a common occurance in these vehicles. When the sun gear fails, you will not have reverse and the forward gears go into "limp home" mode. In limp home mode, the tranmission will only go up to second gear at a high rpm. Best of luck

Check the vacuum hoses the the tranny. The one nearest the bell housing is off or missing.
Im sure its a vacuum leak if the trans is automatic!

Check the ''kick-down'' cable that leads to the throttle body( from the transmission) make sure it returns freely.
