They do not cover "routine eye exams". They do cover exams when there is a medical problem. Examples of medical problems include cataracts, glaucoma, macular disorders, dry eyes, etc.

They cover contacts only if they are a "prosthetic device" as in after cataract surgery when no IOL was used.

They also cover glasses once in your life but only after cataract surgery.

Be aware that "routine eye exam" means that you are having no problems and only want to get new glasses or a checkup to be sure everything is OK. If you have ANY symptoms, things such as blurred vision, light sensitivity, headaches, dry or scratchy eyes, watery eyes, floaters, light flashes, etc, or if you have ever been told that you had a problem such as cataract or that you were a glaucaoma suspect or if you have a family history of serious eye problems or if you have a disease such as high blood pressure or diabetes that can affect your eyes the exam will be covered. If you have any of these report it as the reason for the exam.
