If you took all the media hype away, the best rivalry in baseball is Dodgers and Giants.
Think about it this way. The Dodgers were the lovable losers in New York and people still loved them. The Giants/Yanks also had a nice fanbase however Dodger fans were the hard-working blue-collar fans who would come from work to watch the game.
When they left, the rivalry kept the same intensity on the west coast. Not only that, it's the oldest rivalry as well.

Still Giants and Dodgers. Unlike Cubs and Cardinals, where usually only one is any good, there have been many, many races where both have been in it late in the season -- and even when one is out, there have been occasions where they have spoiled it for the other at the end of the season.
San Francisco and Los Angeles have a rivalry dating back to the Pacific Coast League, and the Giant-Dodger rivalry was very easily transplantable to California, even though the 10-mile difference became a 400-mile difference.

San Francisco-Los Angeles, bar none (began in 1883).
Red Sox-Yankees would come along later. I would say that they are on pretty equal footing.

Mets and Phillies. Teams dislike each other, and most certainly the fans do, as well.
