Diabetic neuropathy pain management?

Question

I have Type II diabetes. It is well controlled and my fasting blood sugar is 90 - 95 mg/dl (~5 - 5.25 mmol/l). I am on a severely restricted low carb diet (less than 50g/day typically). I am 173 cm and 66 kg for a BMI of 22. My body fat percentage is ~17%. But I suffer from severe pain from diabetic neuropathy. I have pins and needles all over, but especially in my hands and feet. I have glove-stocking numbness in one of my calves. I'm not dying. It's what it is. It's not getting any worse, but it's not getting any better. I can't, realistically, do anything differently from a dietary or medicinal perspective. But I would like to alleviate the pain. What is the best course of treatment (and please don't say homeopathy or some such brand of voodoo - it's going to take a lot more than sugar pills and the placebo effect to fix this). Ideally I'd be on a time-release opioid like Oycontin but I can't get it where I live, and there are of course addiction/tolerance factors to consider. There's only so much my doctor can do. I do not want pain to dominate my life.

David, Iwill speak frankly to you. No voodoo. Get a new doctor. A doctor should treat the pain first and then the disease. Period. 

The best medication for neuropathy is a Tri-cyclic anti depressant. Not neurontin. .And a great opiate that is non-addictive is Tramadol. And not Vicodin. Just plain ol' generic Tramadol.
Also add Vitamin B 12 to the list (Mecobalamin) not just plain B12.  
About 1000 mcg. 
I am the same as you, but my pain starts to dominate at about 5 pm. So at 3:30 pm I take 2 Tramadols and 1 hour later I take 1 tramadol. It lasts me the whole nite. And in the day time I can stand the pain, which is less. 
Exercise also helps.

The BEST treatment for Diabetic Neuropathy is longer term blood sugar control.  

Nerves take a LONG time to heal, and also a long time to die.  Because you have neuropathy, this indicates that you have had high blood sugars for a LONG time.  It will take a long time for the nerves to heal, so you must maintain NORMAL blood sugars for MONTHS -- but then again, that IS the goal of diabetes treatment!  In other words -- follow doctors orders, keep your blood sugars under control, and the neuropathywill go away eventually

My feet and legs were hurting so bad and I finally discovered what works for me is 3 Aleve gelcaps, 2 hydrocodone 10/325 and 4 Gabapentin 3 times a day.  It might sound like a lot of meds, but it gets me through my long shifts at work.
