When using a treadmill (Which, by the way is not the best machine to choose to do a workout - you are better off with a cross trainer/ eliptical. Treadmill does a lot of the work for you) you need to vary the elevation and speed to simulate walking/running outdoors. Another good idea to improve fitness is to work in short bursts of speed. IE crank up the pace for a minute, then slow down and then repeat the process for 30 minutes or so. This will improve your overall fitness.
EDIT: HMMMMM is my TD stalker back or does someone not believe the word of a qualified trainer???
Also, a cross trainer has added benefits that a treadmill does not:
1) You can either use your legs to power you or your arms giving greater muscle use.
2) You can "walk" backwards which also uses different muscles than straight running/walking on a treadmill.
Remember: The belt on a treadmill is moving by its self, not by you meaning all you are doing is goin up and down on the spot. An cross trainer is powered by you which means you are using more energy!!!

Elevated running burns more calories, i am a bodybuilder and when it rains i use a treadmill and usually run on it when it is elevated to burn more calories.
