Martial Arts Program Project/Worksheet Questions?

We are implementing an extra program at our martial arts school, for students 12 and older, that focuses on having a good Attitude and Being a Role Model (things we feel are important for our students).

We want them to do a worksheet for each section about a particular word and I need some help creating them or finding already made worksheets.

The words we are planning to use are:

Attitude

Teamwork

Self-Discipline

Courtesy

Confidence

Courage

Appreciation

Charity

Achievement

Can anybody help point me in the right direction or give me some ideas for good questions (maybe short answer) that would be good to use or that you would like to see on a worksheet of this nature?!

    I would never like to see a worksheet, much less a short-answer worksheet, about attitudes or values. These things are best taught by example, by storytelling, by what-if scenarios or by group discussion with topics brought up by the students. If abstract words are to acquire real-life significance, they can't just be filled in on a multiple-choice worksheet (what James Holt used to call "the blip of the blop" or "blip blopths").

    The best martial arts class I ever observed always began with a discussion session. The sensei would say, "First we warm up our minds, then our bodies." The philosophy of aikido permeated every aspect of the teaching and learning. He explained to the parents that teaching and learning are reciprocal. "If they didn't learn it, I can't claim to have taught it." And he emphasized that slow, steady learning is much better than fast learning that hits a plateau and stays there.
