The probability is very low if the casing is properly designed and installed. It could be properly designed but be improperly installed. That would raise the probability of failure considerably.
Only an in depth investigation will reveal what caused the disaster in the Gulf. It could be one or both. or it could be improper operation. Asking it to do something it was not designed for.
