The 12 tribes are the descendants of the 12 sons of Jacob.
They are:
Reuben
Asher
Benjamin
Judah
Levi
Gad
Dan
Naphtali
Issachar
Zebulon
Manassah - son of Joseph
Ephriam - son of Joseph
Joseph was one of Jacob's sons and his favourite - instead of one tribe of Joseph - it was split into 2 tribes for each of Joseph's sons Ephriam and Manassah.
Simon was also a son of Jacob but for some reason one of either Ephriam or Manassah included the descendants of Simon too - they were not their own tribe.
The 12 spies were representatives of each tribe - one man from each tribe was chosen to be a spy. I don't know all of their names, just some of them:
Gibeon
Joshua
Sethur
Joshua was the head of the army of Israel during Moses time as leader of all the people. He ended up being the leader of all the people when Moses died, and he led them on to victory in a number of battles, including Jericho. Gibeon became the head of the army when Joshua took over as leader of the people.
The spys were sent to spy out the land of Canaan - this was the land that God had promised to Abraham, Isaac and Jacob that they would own that land forever, all of their descendants would own this land. The time came for the people of Israel to start taking over some of the land, but there were other cities there. The 12 spies were sent into the land to look it over, check out the people living there, to see if it would be easy to take over the cities, to see how it would be to grow crops, etc.
They brought back a lot of milk, honey, grapes, and other goodies that they managed to get while spying out the land.
Joshua and Gibeon were the two spies that had the most faith in God and were the only two that brought back a good report on the land, while the other 10 spies thought that it would be too hard to take over the land - they didn't have as much faith in God as Gibeon and Joshua - which is why they were chosen as the next leaders.

Tribes of Israel. The tribal arrangement in Israel was based on descent from the 12 sons of Jacob. (Ge 29:32–30:24; 35:16-18) These “twelve family heads [Gr., do′de·ka pa·tri·ar′khas]” produced “the twelve tribes of Israel.” (Ge 49:1-28; Ac 7:8)
Centuries earlier, the prophet Moses described the Promised Land as ‘a land of figs.’ (Deuteronomy 8:8) Twelve spies provided evidence of its fertility by bringing figs and other fruit back to the Israelite camp. (Numbers 13:21-23)

No, the twelve tribes in Rev are symbolic. in case you spot, 2 of the tribes pronounced in Rev weren't component of that unique 12 tribes. And, the Israelites lost prefer with God simply by fact of their rebelliousness. The Jews that make of the a hundred and forty four,000 are religious Jews. (Romans 9:6-8; Matthew 21:40 3)
