Extremely Small baby, what could be the cause?

One of my relatives has a 14 month old baby who was on the small side of normal when she was born, but is now ridiculously tiny. Her weight seems almost proportional to her height, she's a little thin but not unhealthfully so. She does not have any other structural abnormalities except for unusually shaped eyes, similar to that of a child with Down syndrome, but shows no other signs. She is very slightly behind on her milestones.

What could be causing this? At first I feared failure to thrive or some sort of malnutrition, she did seem to reach a height proportional weight after being switched to formula.

To give you a better idea of the size issue, when people see her initially they think she is a doll, she is shorter than a 6 month old baby, and thinner too.

I'm curious as to what could cause this. I have asked the mother about the child's size and she has not been going to well baby check ups and said that WIC said she's a healthy weight for her size, but I think it is quite concerning.

Update:

6 month outfits are a bit too large on her.

Update 2:

She no longer receives WIC, and I think the well baby checks stopped then.

She was actually born a week past due date.

I suppose she could just be petite, but it seems very odd.

    My daughter was like this - she is actually 4 1/2 and can still fit into some 2T clothes. Typically she wears a 3T.

    Some babies are just small/petite. What does her pediatrician say about it?

    With my daughter she was/is 0-3% in height, 15-20% in weight. She isn't failure to thrive, she's just little.

    My daughter is a little like that but not to that extreme. She is 15 months and she wears nine month clothing.

    For her height is in in the 50 percentile and for her weight she is in the 10 percentile. And her doctor isn't concerned at all. She says she is perfectly healthy. And as far as her milestones she is meeting or going way ahead on all of them.

    Since this child is near her milestones then I wouldn't worry so much about weight as of yet. And clearly she been seen regularly by a doctor so that is good. More than likely she will catch up soon enough.

    Potential issues - did the mom smoke or drink, do drugs while pregnant? This could make the baby have developmental issues? How is she getting WIC without well baby care? She also could have something totally random, a case of dwarfism or something that isn't yet noted.

    Unfortunately, since you are not the mom and she is not concerned, short of calling child protective services, there is not much you can do.

    If the baby was born very very early these could be the signs. The baby might be very weak and sensitive. Dont fret because it will be okay.
