I had to do this with my boys. I added small amounts of butter to almost everything they ate (was recommended by their nutritionist)...mashed taters, steamed veggies, scrambled eggs (also added shreddes cheese to eggs while cooking), rice, meats...
Carnation instance breakfast is good by itself. Lots of fattening nutrition.
They didn't really eat any peanut butter, so I don't know what to do with that one.
Avacado's are also good and fatty. Good fats, too.

I know you want recipes, ideas hopefully will be just as good.
Try bisquits an gravy using the carnation for the gravy, instead of milk. You could get some of the instant gravies also, just use the carnation instead of water.
make pancake roll ups. Take cooked pancake, spread peanut butter on it than roll.
You could make a waffle sandwich with eggs an peanut butter, use natural honey instead of syrup.
Search recipes online, and alot of the sites allow you to choose ingredients. Hope this has helped.

Adding protein powder to smoothies for breakfast could be a great source of nutrition.
Oatmeal with fruit (can add peanut butter to thicken it) can work too.
Focus on protein and carbs, not fat.

Wow, slow down Momof3. You just can't come on here and ask that, think of your 21 month old, if your having problems with him or her, speak to your pediatrician please. The other is the community nurse, or your family doctor. Sorry for being blunt, but when it comes to kids they need to be kept healthy, well fed with a nutritious diet, and clean, and I have a thing for the little guys.
