I think it means you have to fight for happiness... nothing comes without a price and most of the time that price is pain. Take the a tattoo for instance for u to enjoy having a tattoo you have to to through the pain for getting it...

I would interpret this saying as pain before pleasure. You could look at it however you want, could mean a whole lot of things depending on how you see it.

maybe pain comes before happiness like to feel happy you need to kn ow what its like to feel pain?
