It is less about where you are from, and more about your ideals and norms of society that you were brought up with.
A lot of Americans are brought up as suburban children, who have nothing but the media to go on as what the rest of the world is like. Ever watch Jerry Springer? They get a lot of those people from low-income and highly-religious areas of the southern USA, and their accent becomes more and more associated with their appearance and personality each time someone watches it.
In short, it's the media. It's a double edged sword. The media makes a lot of ignorant (not stupid, just people who don't know) americans feel like they're entitled to freedom, that they're the best, or that they're truly heroic and patriotic (you see things like cartoons or comics, ie Captain America, etc, or even South Park, with the racial put-downs of everyone who isn't American), and it also portrays the Americans as all self-loving, warmongering, and overly patriotic to others who haven't actually met an american, or been to America. The USA has also been garnering a lot of attention from the "War on Terror", and most of that is just bad press for you guys.
I've met a lot of very intelligent and hard working Americans who truly are deserving of being Captain America, and I've also met a lot of low-class morons who have the american flag in the back of their '82 ford pickup window.
