First avenue I would pursue is to check with the group carrier that has been providing your health insurance. They often have an individual conversion plan. These types of plans usually do not have the level of benefits that you currently have but if you are dealing with preexisting health conditions it might be the way to go.
Second, I would contact the agent that writes your home and auto insurance. A local agent is going to know what the options are available in the private market or through your State.

Check out the site ehealthinsurance.com. (This is not an ad for the site - it is actually the site I have used in the past.) It will give you a listing of all the health insurance plans in your area, including the type (HMO, PPO, etc.), the costs, the company that provides it, etc., and it will let you compare the coverage and rates.
However, if you have questions on your insurance, or problems once you bought it, ehealthinsurance generally won't do anything to help you and tells you you have to find your own salesperson, etc., which is hard to do (and which is what happened to me) since you never dealt directly with that company - you only dealt with ehealthinsurance.com. But on the ehealthinsurance website, you can at least get a list of the insurance that is available in your area. Then you can contact the insurer directly, and if there is a problem or issue or claim or whatever, you have already established a direct relationship with the insurer and they do a lot more to help you than if you went through a third party site.
COBRA is usually your best bet, if you can get it. But once you are over 50, there is no such thing as affordable health insurance.

It is a little difficult to buy health insurance past the age of 60. It is too bad we did not have a Medicare buy in with the Health Care Act for people age 55-64. You should consult with an independent insurance agent in your area who is familiar with the companies and their plans for someone in your situation. If you are low income, you may look into your State health plan. Other than those options, you can always pay cash at a community health clinic.
Here is a web link to some clinics around the country.

Affordable, is a very, VERY subjective term. Your most affordable option, will be COBRA coverage, through your former employer.
If you're in perfect health, not overweight (and 70% of Americans 65 or older ARE overweight) at your age, a REASONABLE price for a comprehensive, low/no deductible plan, is $800 a month. Pricing varies wildly, depending on what state you live in, and which coverages you pick.
You need to talk to a local, independent agent, to tell you what your options are in YOUR state.

Call an insurance broker. You're better off going through a smaller insurance broker, because why use one (named above) that will just sell you a plan and not give you advice or help you after the fact? You pay them and they do nothing in return. A small broker will give you the same plans at the same price and can act as your advocate if you have issues with claims or billing.
