They play overtime quarters one at a time until someone scores. The first to score wins in overtime. Playoff games all play until someone wins. They HAVE to since it is elimination rounds of play. You must play until there is a winner and the loser is eliminated. Either one team wins by making a score, or one team wins when the opposing team concedes. In baseball, this is called extra innings, only instead of "sudden death" as in football, in baseball they play full innings. In football, overtime starts with a coin toss to determine who gets the ball first which is why sudden death works. In baseball they play a full inning so both teams have an equal opportunity to score.

No, because there must be a winner who moves on to the next game. this "5th" quarter is considered a new game and ends at the next score (scoring team wins) in regular season games there is a time limit and if no one scores it ends tied. in playoffs it is played until a score (unless a field goal is kicked. the the opposing team is given another chance. This only applies to playoff games and is void when a touchdown is scored).

Usually if the game is still tied after the first overtime, but I assume they would go to a second overtime in the playoffs because someone needs to win.

No, the video games proceed till there's a winner. a minimum of one activity has long gone 2 OTs simply by this (a Dolphins-Chiefs activity lower back in the early 70s, the longest activity in NFL history).
