High School English: MLA-Format Question?

I am writing a five paragraph essay for a high school English class. My teacher requires us to use MLA formatting to write all of our essays. Our most recent topic is on a book we had to read. My question is if I still need to create a works cited page, considering I only cite one source throughout my essay, which is the book I am reading?

    Some teachers have different rules, but if you don't have time to ask, you min as well make one. It wouldn't hurt to, because you may lose a lot of marks if it was actually required.

    You may as well, it would be best to add a reference, even if it is only one source. It is always helpful if you ever come back to it for revision etc.
