HostGuts.com matches the very close to your budget and expectation. they have pretty good reputation online and i myself host more than 20 websites with them.

For your credit card , if you are looking to accept credit cards from your customers you should signup with a merchant account at a merchant account provider like ccavenue, worldpay etc.

There are a lot of web hosts that will offer you good quality hosting for that price provided you pay for an year or more. If you want to pay monthly then I doubt if you'll get a good quality host that will offer you that price. You'll probably only get low quality hosting where in your site keeps going down every now and then. And of course multiple add-on domains and sub-domains isn't a problem too as they are pretty much standard with a ton of hosts.

I use 2 hosts for my websites: Hostgator and Arvixe and both of them are very good. Their speed, uptime and customer support is very good. Walmart money card is like a VISA debit card I think, so I am doubtful which host accepts that.
