You can only change your name at a court house. You fill out several forms and you will need your birth certificate as proof of your identity. The cost varies by county because you have to pay both a filing fee and for an ad to be placed in the local paper announcing change of name (this is required by law).

Fill out paperwork, file and pay fees. Meet any specific local requirements. (Sherif/fingerprints/whatever)
