Car vibrations at highway speed?

I have a 2001 Volvo S40. Today, as well yesterday I noticed a heavy vibration at higher speeds (80+mph). The car does not vibrate at idle and has no check engine light on. Occasionally the car misses with fast acceleration, however this was not felt while at cruising speed (80-85). In addition the vibration is not consistent. What are possible causes of this?

Wheel balance, u-joints, bad wheel bearings, tire tread separation, bent wheel, and of course engine miss, plugs breaking down at high speed or under load.
