Does a medical consent form need to be notarized?

My mother in law will be watching my children while my husband and I go on vacation. I have drafted a consent form with all the pertinent information. It states the dates effective and lists insurance information as well. My husband and I have signed and dated it, should it still be notarized as well? We want to make sure that in case of emergency, she will be able to take care of things until we arrive.

Update:

I gave her the number to the hospital i prefer and to their pediatrician. I have photocopied our insurance card and attached it as well. I feel so paranoid but its the first time i have left all three of my babies for an extended period of time. Better safe than sorry.

    There is no legal requirement that the document is notarized. However, having it notarized helps confirm the signatures are actually those of the parents, which can't hurt in a medical emergency. Your bank probably has a Notary on staff that can attest to the signatures at no charge. If is preferred that you actually sign in the Notary's presence, but if you physically appear before the notary and state the signature is yours, it should be sufficient in this case.

    no. does she know where to take your kids other than the emergency room such as your regular doctor?

    we have not required notarization for parental consent. Either way, she should have your health insurance card along with the consent.

    What I did when I went out of town with my hubby- I had typed out and printed my own statement of authorization if there was any medical emergency and I also notified my sons doctor so they were aware my sister would be watching him for a week while we were gone. YES notorize this form! You shouldnt have to get a lawyer and a legal form! Good luck!
