Health insurance - even welfare health insurance - doesn't pay for customizing your home. If you don't think he shouldn't have to pay for it, fine. Do it for free.

You personally cannot get it covered now.
To get paid directly, you would have needed to get the paperwork done to make yourself a "provider" before you took the job, and they would have needed to get approval from medicaid before they gave you the job. At this point, it is too late.
Even without you being a provider, it is possible that the customer might be able to get it covered, if they had done things correctly from the start, including certain steps that need to be done before you begin the job. However, if the customer has not already done any of this, then it is probably to late now. Also, you would have to get the customer to pay you, and then the customer would have to get reimbursed by medicare; medicare would not pay you directly, because you are not in their system as a provider.
The customer should have to pay for it, because he decided to hire you instead of following medicare's required process.

Medicare will cover a ramp if a doctor prescribes one as long as it falls within the following guidelines:
It must be durable (long lasting)
used for a medical reason
not usually useful to someone that isn't sick or injured
used in the home
This will include ramps, lifts, bathroom remodeling, and other improvements as long as they follow the previous guidelines.
The doctor and you must be enrolled in Medicare. Doctors and other suppliers have to meet strict standards to enroll and stay enrolled in Medicare. If you are not enrolled, Medicare won’t pay the claim submitted by you.
If you are in California, Florida, Indiana, Kansas, Kentucky, Missouri, North Carolina, Ohio, Pennsylvania, South Carolina, or Texas you may be subject to the new competitive bidding program. You'll find out more information when you contract with Medicare.

Medicare will not pay for a ramp. Medicare covers ONLY medical services and some medical supplies and prescriptions and you have to be a contracted provider for them to reimburse you. The man should have gotten the money first before getting the ramp built, or his daughter should pay for it.
With the lacks of funds in so many states including yours for Medicaid, they certainly wont pay out for a wheelchair ramp. Since 2009 many states have depended on stimulus funds to pay their share of Medicaid. Medicaid is insolvent and you think they will pay for a ramp?
The daughter should do some legwork and contacted some handicapped organizations who might pitch in to pay, or some local charities.

Health insurance does not pay to customize your home. I highly doubt Medicare or Medicaid will pay for it.
If you don't want to do it for free- do it for the price of materials only. No mark up. Consider your labor a donation. Then when you solicit business from companies etc that do have the ability to pay, you can tell them that you volunteer to help the poor disabled and elderly. This will help show that you are a businessman of good character.
You may not get money out of it - but the good will, good karma and marketing are also valuable.

If you do not want to this free, you are advised to get approval from Medicaid that they will pay for this ramp. Contact the local Medicaid folks for further information, fully explaining what it is you intend to do.
