I have an interview with a prestigious consulting firm on Wednesday but my tonsils have swollen to the point where I can't talk and I'm even having trouble with my ear.

How, when, and should I approach my prospective employer about this?

Wait to see what you find out at the clinic, and if it is determined that they need to be removed or otherwise treated, call your prospective employer to arrange a better time. If you are still unable to talk by Tuesday, call them as well.
