I once missed a connection because I went too far away from my flight. I was just trying to find a smoking area. I really thought I had enough time, or I wouldn't have bothered. Not having flown very often, I didn't realize I would have to go back through security on the way back in.

Nope, never.
edit: I usually fly non-stop, or if long distance a flight with only 1 plane change.

Well I have never caught a plane, so therefore the answer is no.
