No, IF you physically disconnect from the internet (ie. disable/unplug your network card so that reconnecting is physically impossible), not just log off.
Otherwise, you leave the (remote) possibility for the connection to be hidden/re-established without your knowledge.

if they were on your home network or a local network that your camera is connected to, then they may be able to see you. If you have disconnected the internet access then no one outside your closed network can see you. If in doubt, unplug the camera or deactivate it in your hardware settings

No chance, unless you have reason to believe it was someone who uses the same computer/ webcam

No-way, and pull the plug too, unless it's on a office network and the link is internal.

yes with very expensive software they could but it would have to be someone professional and also you would have to be very important
