Where/who would I go to to report a place performing unsafe/illegal piercings?

I recently had someone I know take their 7 and 11 year old girls to get there nose pierced which although I know is illegal in my state since in OR you have to be 16 to get a nose piercing even with adult consent, that isn't exactly my problem with it. The main thing is that this place also did it in a very unsafe manner without gloves and sent these little girls home without any aftercare instructions at all so now they now one of them has a pretty badly infected nose piercing. I've been aware of this place before, they pierced my sister without even dotting her face to make sure she liked the placement and once again never wore gloves or showed all of the sterile equipment to us like they are legally obligated to do. It's a newer place but I feel it should be reported right away for all of these things they are doing but the problem is I don't know where you go to even report that.

So my question is where would I report this to and do I have to give names of the little girls or can I just report and be on my way?

    The police department and the health department.

    You shouldn't have to give names of the girls but the police might like to speak to there parents and others you might know of. This helps build and prove a case for shutting down the shop that is preforming illegal piercings.
