Here is one possible way to write it:

Please note that "Mary" is actually written "Marí" because of its pronunciation (Tolkien did the same with "history" in the LotR lower title inscription). A symbol for slash is not attested in Tengwar script, so I used the two dots that are attested as comma, semicolon, colon and full stop.

And I wrote the numbers in our decimal system not in the one originally designed for Tengwar (then the numbers would be 7/74-01/79).
