I was put on Topamax and Amittriptylene both for migraine prophylaxis.?
I have been slowly titrating up to 100 mgs of Topamax and 75 mgs of Amitriptylene. I have felt a slight improvement in the severity of the headaches though not the frequency. It's now been one month I'm on those doses.
What is really bothering me is the shortness of breath and fatigue I feel when I walk a short distance. A hill or one flight of stairs and I'm really heavily breathing.
I would like to know which of the two meds to cut down on. I know they both cause fatigue, but which would make a significant effort if I cut down on? The doctor is not helping.
Anyone been there?

Amatryptiline for me caused more fatigue. I have taken both Topomax did not do anything to me and I would actually take Amatryptiline to sleep, I was taking a high dose though, too high actually. It would knock me out completely I took that up until I starting seizing and was rushed to the hospital and they found out I had serotonin syndrome, so I was taken off of it immediately. Sadly I can no longer take it since I almost died while on it, which sucks cause I got good sleep on it.
That is probably what has caused most of the fatigue you are feeling. Talk to your doctor before lowering the dose!

I would not cut down on either.
The fatigue and shortness of breath are a side effect of the topamax and for most people, they are temporary. I was on both of those medications but at different times, not together. I stopped the Topamax because I felt woozy and disconnected too often. I am now on Inderol and it is, bar none, the best prophylaxis I have ever taken. I have been migraine free for almost 1 year... for the first time in 33 years. Went from having 2-3 a week...to NONE. If the meds are working and are preventing the migraines, but the side effects are too much for you, then talk to your neurologist about it. Many migraine sufferers find that its a matter of trial and error until they find the medication that is right for them. Best of luck!!

I took topamax 5 years ago for my seizure disorder. You will get better. It is just messing around with the chemicals in your brain. Topamax is a strong drug. I had diarrhea at first. Sleep alot. At about week three I started not being able to subtract or balance my check book. I had to have my sister count out my pills (you know how the dose goes up every week). After about 6 weeks everything got better. It also made me not hungry I lost about 50 lbs ( I needed to anyway so that was a plus). I have a seizure disorder and it contolled them. I hear it works for headaches but you will have to be patient. I quit taking it after a couple of years because it made me jittery. I was having road rage and snapping at my son. I think it had just ran it's course which is true in alot of meds. I would take it again if the meds I am on now stopped working.
