Information you are required to provide for being a sperm doner?

I am just curious if you are required to provide a family medical history when you go in to donate sperm?

    Yeah, you have to give your health history and family history to see if any diseases or illnesses are recessive or dominant on their genes.

    And just other stuff.

    yes, they will ask you all kinds of questions,. and u have to spell it donor.
