A horizontally opposed engine is the smoothest, followed by an in-line 6 cylinder and the ever popular V12.
If you look closely at any of these engines you will see that none have an external balancer. They don't require one.

I have an old car fitted with a Rover-derived V8 engine, bored-out just a little to 3.6 litres, with a great 4-barrel Weber carb. It runs so sm00thly, I can stand a One-Pound coin ON-EDGE on the large chrome air-filter, while the engine is running, and it will stay there without rolling away!
