Why do prisons care about hunger strikes?

I've been thinking about successful protesters in prison who go on hunger strikes. Why does it work? What do the prisons care whether their prisoners are eating or not? As long as the prison presents its inmates with the option to eat, then surely they cannot be held accountable for the prisoners choices?

Please clarify what leverage the prisoners have that makes hunger strikes so effective.
