It's likely a burn. That or a blister which can be caused by too humid/toodirty conditions.
You need to keep him clean and put an antiniotic cream without painkiller on it.
If it gets worse then you need to bring him into a vet.

Burns, blisters, scrapes etc etc I'd bring him to a vet, after five years it's time anyway.
You'll need to fix whatever was causing it, it may be something in his environment, and probably just keep the wound clean. Maybe daily soaks in dilute betadine. A cream like polybactrin can help. The vet'll tell you~
