Proper MLA Citations?

Alright, so I'm doing my homework right now and it requires me to find 3 reliable sources for my research paper. I have to fill out these 4 boxes:

Thesis/Main Idea Summary

Types of Information Source Offers

Proper MLA Citations (?)

Reliability of Resource

So I understand what to jot down in each box but the Proper MLA Citations. Do I have to record the address of the resource there or what? Is it asking me if the website has Proper MLA citations? I don't know. Help!

I suppose it's, however you could make a noodle tools account (search it on google) and put in the e-book name and yr published and stuff and then it is going to put it into citation for you, it is fairly helpful.
