Hi Juliet. It isn't possible to say what you should write simply because the answers have got to come from within you. You need to have the basic qualifications for the work and more importantly you must demonstrate a passion and excitement for your future working in mental health. Be confident.
If you possess these elements you should find completing the form reasonably easy. Try asking yourself each question as if you are being interviewed. Then instead of answering vocally write a short paragraph as an answer. Write it as if you was speaking, that way it will come across as being completely natural. The final paragraph of your question is a very good start but you need to build on it.
You appear very keen and you will bring undoubted qualities to the job. Good Luck.
Take care.
