In this case what would happen?

Say there's a fatal four-way for the heavyweight championship and a certain wrestler was supposed to win the title, but he get's so badly injured that he has to be taken out of the match, who wins? Is there a back-up plan, or would the ref get told through an earpiece and then he would tell the wrestlers who would win from there?

Thats how they would do it. Like you described it. In the past, someone along the timekeepers and commentary sitting area use to get told via earpiece. That person would tell the ref who would signal/tell the wrestlers from there who would win.

I'm guessing the ref would be told through the ear piece and somehow signal to the wrestlers who would win, without the rest of us knowing!
