Hypertensive emergency: Get admission to the hospital immediately in order to prevent heart attack, stroke or kidney failure. You have got stage 3 hypertension (severe). Your diastolic BP is very high.
Cardiovascular mortality risk doubles for every 20 mmHg of systolic BP or 10 mmHg of diastolic BP over and above the normal blood pressure of 120/80 mmHg.
Check the function of your kidney (for microalbuminuria, creatinine and blood urea nitrogen), heart (ECG and echocardiography for hypertensive heart disease/ left ventricular hypertrophy) and retina (for hypertensive retinopathy).
Telmisartan is used alone or in combination with other medications to treat high blood pressure. Telmisartan is also used to decrease the chance of heart attack, stroke, or death in people 55 years of age or older who are at high risk for cardiovascular disease. Telmisartan is in a class of medications called angiotensin II receptor antagonists. It works by blocking the action of certain natural substances that tighten the blood vessels, allowing the blood to flow more smoothly and the heart to pump more efficiently.
Angiotensin II receptor antagonist: Common adverse drug reactions (ADRs) include: dizziness, headache, and/or hyperkalemia. Infrequent ADRs associated with therapy include: first dose orthostatic hypotension, rash, diarrhea, dyspepsia, abnormal liver function, muscle cramp, myalgia, back pain, insomnia, decreased hemoglobin levels, renal impairment, pharyngitis, and/or nasal congestion.
Lifestyle changes can help you control and prevent high blood pressure — even if you're taking blood pressure medication.
Here's what you can do:--
* Eat healthy foods.
* Decrease the salt in your diet.
* Maintain a healthy weight.
* Increase physical activity.
* Limit alcohol.
* Don't smoke.
* Manage stress.
* Monitor your blood pressure at home.
* Practice relaxation or slow, deep breathing.
Adopt DASH diet (Mediterranean diet). Eat a low-cholesterol, low-fat diet, which includes cottage cheese, fat-free milk, fish, vegetables, poultry, and egg whites. Use monounsaturated oils such as olive, peanut, and canola oils or polyunsaturated oils such as corn, safflower, soy, sunflower, cottonseed, and soybean oils. Avoid foods with excess fat in them such as meat (especially liver and fatty meat), egg yolks, whole milk, cream, butter, shortening, pastries, cakes, cookies, gravy, peanut butter, chocolate, olives, potato chips, coconut, cheese (other than cottage cheese), coconut oil, palm oil, and fried foods.
Check your lipid profile (Total cholesterol, LDL, HDL and triglyceride).
Consult a cardiologist and a Nephrologist.

In addition to exercise I was able to go off of diabetes medication all together by adopting the DASH diet and losing 170 pounds of weight.
