We have Burger King and McDonalds for fast food. For actual dine-in restaurants, there are a lot of 'american style' restaurants which basically try to imitate American diners, so everything has stars and stripes on and there are pictures of motorcycles everywhere and the decor is always red and the portions are very large :P So yes we have hamburger restaurants but they are usually 'American-style' restaurants and everybody associates the hamburger with America.

We have McDonald's, Burger King, Kentucky Fried Chicken, Subway and used to have Wimpy. I don't know if they all originated in U.S. or not. I'd call these places eateries rather than restaurants.
We have other eating places which are much better without being much more expensive and you get served relatively quickly. You can order a main meal and (whilst it's being prepared) collect salad for your plate and start eating that first or wait for the hot meal to come. You can return for more salad as often as you wish. These places are called Harvester and are excellent value for money and much more comfortable to eat in with much better decor than the real fast food places.
I'm not a great fan of eating out anyway but when I have to do so and, if one is close to where I am, I'd choose a Harvester over all the others any time.
Real restaurants I avoid because I feel out of place in them.

Sure do. They have McD's there (don't know about BK or any of the others though). McD is in Spain and Italy and Japan and a lot of other countries although some of them have a slight variation on some of the stuffings. However, one reason I travel to other cultures is to eat their food so going to McD seems like a silly thing to do--I can have McD at home.

would not call them restaurants but fast food cafes,to me a restaurant is where you sit down and are waited upon whereas in a hamburger shop you queue as in a cafeteria.

That is a very short-sighted view of American restaurants. Hamburger is the food of the commoner even though most Americans love them. We do have elegant restaurants that would put the boiled beef folks on the other side of the pond to shame. If we are known for anything it is our steak, fresh water fish, multitudes of fresh vegetables. We have an amalgam of foods brought from other countries starting with the hamburger which started out in Europe where the toughest cuts of meat were chopped up for people who couldn't afford the finer cuts. With the toughest cuts, you had a choice: grind them up or boil them all day to make them tender enough to eat. We perfected it and began using better cuts of meat to make it. The other "american" staple, hot dogs, came from Germany where sausage was king!
