No, they aren't dead. I watched again. He keeps injecting her to make her seem dead when people are around. You can't inject a dead body, there would be no blood flow to carry the drug through the body.

The way I interpreted it, she wasn't dead yet. He was observing her before at the funeral and seemed to know that her life was empty and miserable, that she wasn't happy at all. Maybe he felt that she wasn't living her life properly and hence didn't deserve to live at all. Otherwise, why would he be following her all the way till her accident happened, and did nothing to help at all, other than just wait and observe?

Yep, the mirror is a puzzler. But they are dead, they just do not know it. Few people get a second chance, and essentially no one on the mortician table fits that category.
