I have to turn in a worksheet to the insuance company for repairs on my home what form do i use?

Update:

The insurance company wants man hours how much,,, a list of material in every aspect of the job... from roof to basement.. They dont give you a form.. You have to get one .. I just cant find a site..

    If they dont give you one then type one up listing what it is and the actual cost so they will have to pay you the original price it cost you if you had a full replacement policy, if not they will try to itemize it. Example something you paid $1,000 for 10 years ago they will only offer $100 though it might be worth lots more today.

    Hang on while I call the insurance company and ask them. Oh, wait. I have no clue which of the many companies you might be dealing with. Maybe you could call and ask yourself.
