New patient forms- name of insured?
I am looking to set up an appointment with a GYN I have never visited one before. I have looked at several and on their new patient forms, they have a section for "name of insured" which requires a SS#. From my understanding, the name of insured would be my dad's name because it is provided by his work. I have an insurance card, is that not good enough? I would rather not ask my dad to fill out a GYN form.

Having worked in a doctor's office before, I can tell you that the forms are like that because, in order to bill the insurance company, they need to have the name, date of birth and social security number of the primary insured (who has the main policy).
Just tell you Dad that you have a doctor's appointment coming up and you need the information for the forms. If he is not comfortable with giving it to you, call the doctor's office and explain the situation. They may be willing to work with you if you provide a phone number for the main insured where they can obtain the information from him directly.
Another option would be to check and see if the doctor's office has a web page and if the forms are available there. If so, bring up a set and fill out name, address, etc. for yourself and stop there. Then call out, "Hey Dad! I need your help with this for a minute," and ask him to type that info in directly. Problem solved.

I would call the doctor's office and tell them you are covered by dad's insurance policy and have the card. Ask if they still need the number. They may not since you have the card. Those forms are all standard and questions like that are nice to have answered but may not be required.
