Visitors visa from MEX to USA?

Hey guys my cousin from mexico just got her visitor's visa and passport from mexico to come visit me in Cali. We have family in Colorado and Illinois so im wondering if her 10 year visa with temporary stays of 6 months is good for all three states? In her interview she said she was going to see me here in Los Angeles.

Question: Is it ok if she enters to california by car and( at the immigration line they give her a stamp or seal,) and by car traveles to other states??

Question: if she can, is it okay if she leaves from illinois via airplane back to mexico? I remember that when you enter a different country they give you a little paper stapled to your passport and when you return they take it away. She wont have that since shes entering by car through california.

Question: how does she get that visa paper?

Question: if she doesnt have it and leaves via plane will they charge her a fee?

    Visa is valid for the whole country.

    She can travel via car or plane through whole US.

    Yes she can fly back to Mexico from Illinois.

    She will be issued a form I-94 when she tries to enter US, which she will need to fill out and give a customs officer at point of entry (airport, seaport, or land checkpoint). If the officer thinks everything checks out he will stamp it and stamp her passport. When she'll be leaving US she'll have to surrender the form to another customs officer.

    If she loses the I-94.

    She will be allowed to leave, however there might be problems if she tries to re-enter US later. If the form is lost a replacement can be request using the I-102 form (which costs 330 dollars). So it's best to just keep the form somewhere safe, it will not be used except when entering and leaving anyways.

    The visa is good for all states--no restrictions. She can enter by car. Upon entry she will be given

    an entry document ( form I-94) with an endorsement as to the time allowed. Upon departure by plane, the form will be lifted. If the form is lost, there is no fee.
