Transcript Help.......?

Question

Where can I get a transcript template?

Also, I'm a bit confused. In my transcript, do I just write 'English,' or should I be more specific, like 'Native American Literature'?

You will want to be specific, showing by grade level.  I didn't use Excel for my daughter's, just a Word document.  I used school "letterhead," name your school.  The grade and classes taken, the grade given with grade point average.  Dates of any tests taken, my daughter happened to have taken the CHSPE.  Any volunteer work, etc.  Oh, at the top put student name, address, DOB.  
Then a parent signs, I put this...

Parental Certification:		
I, _____________, do hereby certify and affirm that this is the official transcript and record of _______________ in the academic studies of (dates).  With signature after it.
