Help ! with a android T-Mobile?

I have a android T-Mobile phone.how do I unlock the PUK code? I can't received texts or calls and I tried to go to the T-Mobile page but I forgot my password and username? please help

    Your need to phone them up. You may have to verify your ages and identy before they hand your puk code over.
