It is best to call or go into your phone providers store as they should be able to send or give you one then and there for free. Phone provider as in Orange, EE, Vodaphone, O2 etc.

You would have to call Phones 4 U to find out as it doesn't say anywhere that they do (which makes me think that they wouldn't)

A properly equipped store will CUT the SIM card to size for you. Don't worry, if you keep the outer piece you can still fit it in your old phone.

you can easily get the sim cut by a nearby mobile seller or may be yourself. the i4s uses micro sim but. cutting a sim will not void the contract. since you'll still be using the same sim. only extra plastic will be cut from it.
