Your hardest Mass Effect mission, where you died the most?
On Mass Effect 3, I got killed 13 times on that last mission where you (with Javik and Garrus) fight by the missile trucks, yet I died 11 times during the rest of the game combined.
Now, as an ME veteran who has played Mass Effect 2 twice (once on veteran level), and Mass Effect 3 once, I got killed 18 times on Mass Effect 1's Therum mission. It's the one where you spend the first half in the 6 wheel Mako, then the rest of it in the caves looking for Liara.
My problem was I (with Garrus and Tali) did that one real early, and didn't have high enough skills built up.
Mass Effect 2's Horizon mission (with Mordin and Garrus) and Derelict Reaper mission (with Tali and Grunt) were the hardest for me, but I only died maybe twice each.
For you ME fans, which missions were the most troublesome back then?

Well I’ve run insanity on every class now multiple times. Consistently problematic would have to be ME2 Archangel mission where you have to shut 3 doors. Door #3 was always hell for me on insanity regardless of class. The Krogan weren’t that bad, but the Vorcha force you to stay in cover, and the virmin always manage to sneak around and attack you from the rear. I always left 1 squadmember with Garrus, which just makes it harder.
ME3 final mission where you have to fire the rockets was also hell on insanity, but strong biotic group /w Warp debuffs made it a lot easier compared to something like a Soldier or Vanguard.

This is a tough question. I would say that during my Insanity playthrough, the hardest mission (not including any side missions, didn't do them) The last mission was the hardest, especially right at the end when you're fighting the brute and the banshees. However, I cannot play through Mass Effect 2 on insanity, it's just too difficult for me. So for that reason, I would say the toughest mission is any of the collector missions in ME2

In ME1 I died quite often on Therum, which has a difficult fight against the geth right before the dig site, and later against that krogan.
In ME2 I thought the fight against the scions and praetorian on Horizon was quite difficult.
In ME3, I didn't really think any fight in particular was difficult, even on insanity difficulty. It was clearly the easiest game in the series. If I have to pick, I'd say fighting Cerberus waves on Benning, but only because it was a long fight. The final fight on Earth was intense, but it's timed, so you can just keep avoiding the enemies until you're ready to fire the missiles. Playing as infiltrator makes it especially easy.
