My brother is bipolar, so seen this for numerous years. Make sure he is taking his medication, even when he feels great. Try to minimise the damage from both extremes. You'll need a very thick skin, you'll be blamed for everything and be a super hero too. Get your sea sickness tablets. "It's gonna be a bumpy ride".
