Mass Effect 2 - Did someone die during my suicide mission?
This will be the third or fourth time I've beaten this game so I'm pretty sure I did everything right. Everyone was loyal. I took Tali & Miranda with me for the final battle and during the cutscene when Shepard contacts the team that stayed behind to guard the door, I see someone get shot and fall on the ground.
Back on the ship however, it doesn't show any coffin or anything. Did someone die? Is there anyway to check?

Some very inaccurate answers, even from top contributors. Time to set this right.
First of all, to all other answerers, the question is no "why did someone die", but "did someone die." The answer is a simple "no", as the falling animation is something that sometimes happens during this cutscene, but it doesn't mean the character is death. It's a simple detail in the backrgound. If someone did die, then when contacting Joker after the final boss, you see their corpse on the floor.
Now that I answered the question, I want to talk briefly about the death conditions in this phase, to counter the high amount of misinformation given by other users.
Deaths in the final phase are determined by defense scores and present characters. Strong characters, like Garrus, Grunt and Zaeed, have a defensive score of 3 while weak characters, like Mordin, Jack, Tali and Kasumi have a score of 0. Everyone else has a score of 1. Add one if the character is loyal. It then takes the average score of the squadmates guarding the door, and if that score is at least 2, no one dies during this phase. Contrary to what D.B. Cooper says, time spent fighting the boss is not a relevant factor, nor is any decision from a previous phase in the suicide mission, except the indirect effects of making a good defender crew escort (in his case Zaeed). All that matters is the math I described earlier, which you can review in the image I posted.
Contrary to what Alex and Invalid Username say, deaths in the suicide mission don't happen without a reason. It's all calculated. Even loyal characters can die.
Contrary to what matthew says, it's possible that some, but not all, squadmates at the door die. It's no all or nothing situation.

Mordin dead at the very end happened once for me. It had to have happened because:
1. Took too long killing the human reaper.
2. Named Jacob as the other squad leader.
3. Sent Zaeed as escort, taking away some firepower.
I reloaded the save, and did it differently. I never had anyone die when I:
1. Named Garrus as 2nd Squad Leader.
2. Sent Kasumi through the vents.
3. Took Legion and Jack with me to open the vents.
4. Samara as my biotic escort.
5. Miranda as 2nd Squad Leader after the two teams met up.
6. Thane and Zaeed with me and Samara.
7. Tali on prisoner escort to ship.
8. Grunt and Legion with me on the final battle.
One way to know if everyone lived is to check your ship after the credits have rolled:
1. Are all 12 still around?
2. Kelli and Sgt. Gardner?
3. Ken and Gabby?
4. Two crew members at the table in the bunk bed area?
5. The woman still at the kitchen table, but the man should be on a cot in the Medical Bay.
When you are done with the suicide mission, everyone should have something new to say.
You should also visit the Shadow Broker Base one last time for Legion's dossier, and fresh shipments.

Probably a glitch. When Shepard contacts the door team, they're either just fine or completely wiped out, nothing in between. If you're unsure, just check around the Normandy after the credits and see if anyone is missing.

The only person I've seen die in the final scene is always Mordin Solus. It might be him. Sometimes he dies for no reason on that part when you're about to activate the bomb, talking to Joker. It's happend to me at least two times now.

It all depends on your relationship to the crew if its good then no one dies well sometimes it glitches and someone dies for no reason but for the most part it all depends on your relationship. And nearly all your crew dies besides your team. usually at the end it should tell you.
